A New Life Without Back Pain
“We can’t go on like this, Ms. M. – I’m referring you to an esteemed colleague in Cologne to take a closer look at your back.” With these words from my orthopedist, a new chapter began in my long history of suffering.
At that point, I was 45 years old and had already undergone two disc surgeries. I had exhausted every conservative treatment imaginable: physiotherapy, injections, acupuncture, pain management therapy, "back school," rehabilitation sports, and even alternative healing methods. Since my first herniated disc at age 29, more than 16 years had passed—years defined by chronic back pain.
Every morning I woke up in pain; every night I went to bed with it. I felt exhausted and hopeless, often feeling much older than my years. I had tried everything known to me, but nothing provided lasting relief. That changed the day I sat in the spine consultation at the Sana Clinic with Dr. Biren Desai.

Trust Built on Empathy and Expertise
That first meeting was a turning point. Dr. Desai took his time, listened intently, and showed a level of empathy that I hadn't experienced before. For the first time, I felt truly heard and taken seriously. He gave me something I hadn't felt in a long time: hope.
Initially, we opted for periradicular therapy (PRT)—a targeted injection to the nerve root. However, when the success was only moderate, we sat down again to discuss the possibility of artificial disc replacement. I remember his detailed explanation and his patience in addressing the fear that was clearly written on my face. For the first time, I saw a real perspective—a chance to finally leave my suffering behind.
Despite the seriousness of the situation, there was room for humor. While looking at my MRI scans, I mentioned that discs need movement and water to stay healthy. Dr. Desai quipped, “Yes, Ms. M. – provided one actually has any left.” We both laughed, as the images showed almost no visible disc material remaining. This blend of high-level competence and human warmth made it easy for me to trust him. I even kept the sketch he drew during our consultation to explain the surgical procedure—it remains a memento of that turning point.
The Surgery and the Path Back to Activity
In March 2016, the procedure took place. I received two artificial discs in my lower back (L4/L5 and L5/S1). The surgery went perfectly, and I felt exceptionally well cared for throughout my stay. Dr. Desai kept his word and called my family immediately after the operation to let them know everything had gone well.
Just five days later, I was able to leave the hospital. The healing process was seamless, and the follow-up care was outstanding. Six months later, I proudly held the certificate of my first completed triathlon in my hands and sent a photo to Dr. Desai. It was my way of showing him what his treatment had made possible.
Nine Years of Freedom
Today, nine years later, I am doing wonderfully. Thanks to that surgery, I live a pain-free life. I engage in strength training, running, swimming, and squash. I go hiking and travel the world with a backpack—doing everything I want to do without limitations.
